What this policy covers

This policy covers information collected through this website and our SMS (text message) service. Protected health information created or received in the course of your treatment is governed by our Notice of Privacy Practices.

Information we collect on this website

• Information you give us: your name, email address, phone number, the state you will be in during a visit, your treatment interest, your appointment choice and your text-message consent when you complete our assessment, booking or membership forms.
• Account information: if you create a member account, your sign-in email, an encrypted password and your membership and payment records.
• Payment information: payments are processed on our payment processor’s secure page. We do not receive or store your full card number.
• Technical information: IP address, browser and device type and pages visited, collected through server logs to operate and secure the site.

How we use it

• To respond to your assessment, appointment request or inquiry
• To schedule, confirm and remind you about appointments
• To manage memberships and payments
• To operate, secure and improve the website
• To meet our legal and regulatory obligations

How we share it

We do not sell your personal information. We share it only with service providers who help us operate the practice (such as scheduling, communications, payment processing and hosting) under contracts that require them to protect it. Where those providers handle protected health information, we require a business associate agreement. We may disclose information when required by law.

Security

We use administrative, technical and physical safeguards to protect information. No method of transmission over the internet is completely secure, so please do not send detailed medical information through website forms or regular email.

Your choices

• Reply STOP to any text message to stop receiving texts.

Text message (SMS) privacy

Health Haven and its affiliates (collectively, “Health Haven”) are committed to protecting your privacy. This section governs how we treat the personal information we collect and receive from you in connection with your use of the SMS service (“Service”), which we make available through a third-party service provider.

“Personal information” means information that individually identifies you, such as your mobile phone number or user name, as well as any personal information you choose to include in messages you send through the Service.

Opting in

You may opt in by entering your phone number online, sending a mobile-originated message containing a keyword, or filling out a paper form that includes your phone number. Carriers are not liable for delayed or undelivered messages. Message and data rates may apply. If you have questions about your text or data plan, contact your wireless provider.

Opting out

You can opt out at any time by texting “STOP.” We will send one confirmation message, and you will then receive no further messages from us. To rejoin, sign up again as you did the first time. We may still send certain administrative communications related to the Service.

Children

The Service is not intended for children under 18, and Health Haven does not knowingly collect information from children under 18. By using the Service, you represent that you are at least 18 years old or that you are under 18 and have your parent’s or guardian’s permission.
